Data Backup Logs

                  The data backup log consists of a customer copy page as well as a technician or store copy page. This form is great for documenting the data that is to be backed up as well as for recording notes regarding the actual backup process. This form is useable in the physical store environment as well as for on location service. Additionally, this form can be used in conjunction with the maintenance contract logs to document the success or failure of various backup types. Data Backup Log

Computer Information and System Specifications:
Although not different than the Computer Information and System Specifications section of the other forms it is still important to document the vitals of the computer that is being backed up.  This will ensure that this form is tied to any other services performed on the computer in question. 

Common Backup Items:
Use this simple check box section to document what items are to be backed up as part of the backup service. There is also a check box that is used to communicate if that particular component of the back up successful.

Location of Additional Items Requested:
This is a free form section allowing you to document any addition items that the customer may have request for backup. This is handy for documenting non-standard request or for documenting the backup of data stored somewhere on the computer other then the typical user folders.

Acceptance of the Terms of Service:
Capture the customers signature prior to performing the actual backup to ensure that they accept the Data Backup Terms of Service

Backup From, Backup To, and Type of Backup:
These three section provide further information about the type of backup to be performed. Here you can document where the source files are located, where the backup is to be stored and the type of backup being performed.

Backup Terms of Service:
It is important to get specific acknowledgement that the customer accepts the Backup Terms of Service even if your Terms of Service already cover the backup process. This will also allow you to have more a Backup Terms of Service that is more in depth and able to cover things such as automated data backup services or data archival services. As always you will want to consult your lawyer before utilizing any Terms of Service to ensure that they are valid for your particular business and your particular situation.

This is a free form section that will allow you to document the entire backup service.

Data Backup Restoration Service:
This is an informational section that informs the client that additional changes or service fees may be assessed for data restoration or retrieval services.
