This form actually consists of two Maintenance Contract logs than can also be adapted to other uses. With a few modifications you could use these forms as the foundation for a site assessment. The non-modified versions are classified as a Service Contract: Quarterly Maintenance Log and as a Computer Maintenance Log. Site Tech Specialists use a slightly modified (replaced computer Information with site information) version of the Quarterly Maintenance log to provide a high level overview of the maintenance visits performed over the quarter. A Computer Maintenance Log is kept for each computer covered by the contract and is used to record each service incident. Multiple copies of this form may be needed depending on the number of service incidents for that particular computer. Finally, the maintenance forms are used in conjunction with work order forms as needed.  This creates a hierarchy of documentation making it very easy to drill down on individual service calls. With our maintenance contract customers we maintain the quarterly log and computer log at our location and utilize work orders at the clients location. This makes it easy for us to update the logs when non-physical access incidents (phone support and remote support) are completed. Each quarter, at a minimum, we send PDF and hard copies of the logs and work orders to our clients detailing their service history and total savings.
